Online Paperwork

We politely ask that you take the time to fill out our new patient intake ahead of time, to save you time at your first visit. 

This intake will give us a better understanding of your health history and your current condition so that we can determine the best course of action.

Once you schedule your New Patient Exam you will receive an intake form via email upon signing up for your first appointment.

If you are unable to fill out the paperwork before coming into the office, we ask that you please arrive to your appointment 15 minutes early to fill out the paperwork. 

Consultation

Your doctor will go through your paperwork before beginning the consultation to help better understand your case. The doctors will then sit-down with you in office and get the chance to meet you as well as hear about your case.

The consultation can take anywhere from 10-20 minutes and is meant for the doctors to find out more about your condition.

We will ask a variety of questions that pertain to you and your case. Most importantly, we establish YOUR GOALS of care. 

Looking to just get out of pain? Be able to go workout? Pick up and play with your kids? Every patient comes in with different goals of care, and we look to setup a treatment plan that makes the most sense for you!

We love getting to know our patients! We not only want to get to know about your case, but about your family, job, etc. We want to create an atmosphere where patients can be open with us and enjoy their time in our office!

Examination

Patients can expect to be active and involved during the examination as the doctors will put you through a variety of different tests to help further determine our diagnosis for you. The doctors may ask you to put on a gown for parts of the exam. A few parts of the exam will include range of motion, neurological & orthopedic exams, postural & gait analysis and more!

The most important thing for us is that our treatments are tailored to each individual. We are not a cookie-cutter clinic. It is important to us that our patients feel as comfortable and informed as they can possibly be during each visit. We want to get to the root of the cause of your injury and find ways to get you feeling better!

Treatment

Our goal is to get you feeling better as quickly as possible. At your intial visit, you will receive some form of treatment that is best fit for your condition. Whether that is an adjustment, soft tissue work, or passive stretching, we want to be able to provide you with relief at your first visit before establishing a treatment plan. 

We offer many therapies such as myofacsial release techniques, cupping, kinesio-taping, mechanical massage, and instrument-assisted soft tissue mobilization. We also create and email you an RX report within 48 hours of your first visit. This includes information about your diagnosis established during your first visit, descriptions and reasoning for treatment methods your doctor has prescribed, lifestyle modifications, and video access to at-home exercises prescribed specifically to you.

We want to give our patients autonomy in making decisions for yourselves (with our guidance) throughout your first visit. Never feel like you're going to be sold on one sort of treatment option. We recogonize different options work for different people and different conditions, so we want to be sure you feel heard while also are receiving the most effective treatment possible!

Payment Options

We offer flexible self-payment options such as HSA, FSA, as well as CareCredit. Traditional methods such as cash, credit/debit card are also accepted.

Have an out of network insurance that you want to utilize? Ask us about verifying your benefits and we will explore your options.

Upon request we can send you home with a superbill that allows you to be reimbursed through your network insurance directly!
